A case manager is a healthcare professional who helps patients and their families and/or caregivers get through tough times. A case manager can give advice and help patients who have disabilities, mental health issues or social needs find services they need to maintain optimal health and quality of life. He/she also works closely with doctors, social workers and other community service providers to ensure patient needs are met. As a Chen Medical Centers patient, we can link you to Community Connections services, which include support from a case manager, if life’s challenges may be affecting how you live. Your case manager can help you link to community agencies for help securing food, shelter, in-home support and other needed services. He/she can also help you prepare for a hospital discharge and your aftercare needs, connect you to support groups and recommend activity programs that improve your health. Your case manager will help guide you along the way and follow up for ongoing support.

Your PCP will also work closely with a skin specialist (called a dermatologist) to coordinate your personal care plan and help keep your skin, hair and nails their healthiest. Gastroenterology is a medical specialty that focuses on the treatment of the digestive system, known as the gastrointestinal tract. This is often referred to as your “GI” tract and includes all of the body’s organs that are involved in digestive processes such as the esophagus, stomach, small intestine, colon and rectum, pancreas, gallbladder, bile ducts and liver. Your digestive tract helps the body absorb nutrients and eliminate waste. Many types of illnesses like infections from bacteria, viruses or parasites can cause GI problems. This is a term for hearing loss. It means a person has trouble hearing in one or both ears. It can also mean that a person cannot hear at all (is deaf) in one or both ears. Your PCP may recommend you see a hearing specialist if your issue requires it. A hospitalist is a medical doctor who works full-time in a hospital. As a Chen Medical Centers patient, you never have to worry about being alone in the hospital. Chen Medical Centers has hospitalists onsite that oversee your care during your stay. This doctor is available to you at all times and keeps your primary care physician and specialists notified and up to date on your condition and coordinates all of your in-patient care until you go home.
An echocardiogram (or “echo” for short) is a sonogram of your heart. An echo is a test that uses ultrasound waves to take a picture of the heart and its other structures. An echo can provide your doctor with useful information, such as the size and shape of your heart. This test can also help your doctor measure how well your heart is working, check for fluid around the heart and locate defects or infections. At Chen Medical Centers, we perform echocardiograms right in our centers, so you don’t have to travel to a diagnostic center for this important test.

Nuclear Stress Test
A Nuclear stress test helps to determine if there are any blockages in the vessels that provide blood to the heart. It measures blood flow while the heart is at rest and compares it to when the heart is working harder as a result of exertion (exercise) or medication. Images are taken that are able to show damaged heart muscle or where there may be decreased blood flow through the heart. We offer this test right here in our centers for your convenience.
A nutritionist is a healthcare professional who specializes in food and nutrition. As an extension of our cardiology services, Chen Senior Medical Center has relationships with many different pacemaker manufacturers. Technicians from these manufacturers come onsite when cardiologists are in the office to perform pacemaker checks. After the technician checks the pacemaker our cardiologist will review the results before the patient leaves the office. The check itself takes 15-45 minutes, and includes testing the pacemaker's battery and ensuring heart rate and heart rhythm data are being collected properly.
Podiatry is a medical specialty that focuses on the diagnosis and treatment of your feet, ankles and lower extremeties. This includes your lungs and the passageways that oxygen travels through to get to your lungs, such as your throat and sinuses. A pulmonologist is trained in diseases and conditions of the chest, particularly pneumonia, asthma, tuberculosis, emphysema and other chest infections. This medical specialty uses radiation through tests like X-rays and ultrasound to diagnose and treat certain health problems and diseases. Chen Medical Centers provides many different diagnostic services right in our centers so you don’t have to go to multiple places to get them done. Our services include EKGs to test your heart, lab work such as urinalysis, blood work, flu and strep tests, X-rays, ultrasounds and more. Skilled nursing is a level of care given when a patient needs full-time care from a nurse or other skilled nursing professional (SNP) to help the patient recover, especially after surgery, an injury or a hospital stay. This level of care may be required for short- or long-term needs. A skilled nursing professional can help manage, observe and evaluate your care outside of a medical office or hospital—usually in the home or a skilled nursing facility like a nursing home. Examples of skilled nursing care include IV injections, physical therapy and personal care like help with bathing, dressing and walking if you can no longer do them by yourself. Patients who have Medicare can get skilled nursing care under certain conditions.
The social worker is a non-medical specialist who can assist patients with linkage to community resources. This role will advocate for the social needs of the patient and assist the medical team with identifying resources that may help the patient remain independent and healthy. The social worker will assess the strengths of the individual, family, and social support systems to help patients function and remain independent within their communities. Community resources include Alliance for Aging, Meals on Wheels, Senior Community Centers, Adult Day Care Programs and Community Mental Health Centers.

Wound care is when a healthcare professional helps your body go through its normal healing process after it is damaged by things like surgery, a sore, cut, break or other injury.
