Patient Experience Survey and Ratings Guide

As part of our commitment to patient listening, we regularly survey our patients to assess our effectiveness at delivering exceptional service experiences. Our survey performance ratings and open-ended comments are posted online to help our patients and community members make informed choices regarding their medical care.

About Our Survey

To ensure we hold ourselves to the highest ethical standards, we’ve outsourced our patient survey process to Qualtrics, an independent and widely respected experience management company that specializes in the collection and analysis of customer feedback.

What Types of Questions Are Asked?

The survey invites patients to evaluate us across a range of service factors relative to their most recent on-site or virtual care visit, including appointment scheduling, wait time experience, and care provider interaction.

Star ratings are calculated by averaging the numerical responses to the following questions:

  • Likely to recommend to friends or family?
  • Amount of time spent with you?
  • Trust to take care of your needs?
  • Likely to continue using in the future?
  • Ability to listen carefully to you?
  • Overall care received?
  • Cares about you as a person?

Who Can Complete a Survey or Post a Comment?

Only patients/caretakers are allowed to complete a survey and provide survey comments. The results are based solely on patients who have received direct care from one of our centers.

How is Patient Information Safeguarded?

Patient names or related identifiers are removed prior to online display.
